This Form will be sent by PM only. If it best to send a copy to the Tester and an Admin so that they can both approve of the retest. They will send a PM back regarding the status of the test.

2. Why do you feel the member should be retested for dorm? - At least 1 Paragraph

Please be extremely detailed when writing this form. Anything that seems like an invalid reason or a weak statement will resolve in automatic denial. This is not to be abused, and as such the rules are as followed:

- You can only send in a Credibility Form after a week of the test.
- You may only send 1 request per month. This is so that people won't get too overboard with new members of the Dorm.
- If the tester, for any reason, declines the form, it is still considered a request, and your "1 Request Per Month" will take effect.
